Lost my DP Slips Please help

Hi All,
Currently I'm with IIFL and now moving to another one. Hence want to transfer my current shares to new one. However the I have lost the forms that IIFL provided me earlier. Therefore please help me as how I can move my shares to the new agency.

Are there any online forms available for download, please provide the links.
Should I contact the IIFL Office and will they provide me the duplicates/fresh forms.

Please help.

Do contact your DP office and they will guide you. You can also get in touch with NSDL or CDSL and seek guidance.
