Need a Scanned copy of the failed certificate

Hello Friends,

Can any one of you provide me the the scanned copy of the printout that we get when we are finished with the exam.

Please note that I want the scan of the failed persons. If thats not possible then the format in which the printout is given will be useful.
Again it should of the person who has failed.
